YOUR OFFICE VISIT

We see patients by appointment in order to reserve ample time for examination and discussion. If you have an urgent need to be seen, please call our office. We will do our best to work you into the day’s schedule. To better accommodate our patients, we generally open our office at 7:30 a.m.

IN CASE OF EMERGENCY

FEES AND INSURANCE

We are committed to providing quality eye care at an affordable cost. Because billing and other administrative expenses can increase our fees, we encourage payment for your office visit at the time of service. You will be provided with an itemized receipt which may be attached to your claim for insurance benefits. We are providers under most insurance programs, including Blue Choice and MVP. You may find it convenient to download and complete the Insurance Form and Privacy Form (on next page) that we are required to keep on file, and bring them to your office visit.
